Look for the image like the one above in the slide on Sam’s Club website.
Click “get a sample”. A form will come up, fill in your mailing info and it will be on the way.
It does require a membership number but you can sign up on Sam’s Club website as a guest.
If you sign up as a guest, you will get a guest member number. Find your number in your account info.
